COVID-19 Vaccination Requirement
Everyone attending the SFJAZZ Gala will be required to show proof of vaccination and a government issued photo ID. Boosters are encouraged for eligible individuals, but not required.
If you received your vaccine in the state of California, you can find a digital proof of your record here. We recommend saving the digital record on a phone or mobile device.
COVID-19 Testing Requirement
Everyone attending the SFJAZZ Gala will also be required to show proof of a negative COVID-19 rapid antigen test taken within 24 hours of the event, or a negative PCR test taken within 48 hours of the event. At-home test kits are also acceptable.
What testing proof do I need to provide?
Simply take a selfie with your negative test result and present the photo at check-in, along with your proof of vaccination and photo ID. Please note that we cannot accept results emailed in advance.

SFJAZZ Center
Tests are also available for Gala attendees and can be picked up at the SFJAZZ Box Office during regular operating hours: Tuesday - Saturday at 11am - 5:30pm.
Note: The Box Office will remain closed on May 24-25.
